Alcohol Questionnaire
Alcohol Questionnaire for Adults
Please answer the questions in the short questionnaire to enable us to assess your alcohol consumption rate and offer advice if necessary.
Asthma Review
For patients who are due an annual asthma review.
Please would you answer the questions on the form below and submit it to us.
If your symptoms are deteriorating or you have any concerns, please make an appointment to the respiratory nurse or a doctor as well.

Non-NHS Services
Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
- Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
- Insurance claim forms
- Passport signing
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
Anticoagulation Monitoring
If you are taking warfarin tablets our practice nurse will take your blood here in the surgery so that you do not have to go to the hospital for your blood test.
Health Checks
These are available for patients not seen for 3 years and the over 75’s.
Flu Vaccination
In accordance with the Department of Health guidelines, we recommend an influenza vaccine for the elderly and patients with chronic heart, lung or kidney disease, diabetes and residents of nursing and rest homes. The vaccination is usually available in October/November.

Travel Vaccinations and Foreign Travel
The Practice offers advice and vaccinations for travel abroad. Most of the vaccinations are free on the NHS. We do charge for the vaccinations not available on the NHS and for certificates.
Before Travelling please make sure to:
- Give 2 – 4 weeks’ notice to enable us to complete programmes and to allow Vaccines to be ordered and work as intended

Minor Surgery
A variety of small operations and joint injections are performed by the doctors in the surgery. If the doctor recommends this a special appointment will be made. There is a minor surgery clinic once a month on Thursday morning.
Cervical Smears
These are done by the practice nurse. Please inform the reception staff when you book your appointment so that extra time can be allowed in surgery. Our policy is to test every woman aged between twenty five years and sixty five years every 3 or 5 years—unless the consultant or laboratory recommends repeating the smear more often.
