Please tell the receptionist that you want to register with this Practice and you will be given registration forms to fill in.
You will also be asked to provide identification:
Acceptable Forms of Identification
- Photo ID: Passport, Drivers Liscence etc
- Utility Bill confirming your Address
If you change your name, address or telephone number, please let our receptionists know straight away . If you move outside the practice area (see map above) it will be necessary for you to find a doctor in your new area.
